You can complete the Authorization Agreement for Direct Deposit of Payroll if you haven't enrolled in direct deposit. You will need to download, print, and complete this form. Please have the following information ready when completing the form:

·       Social Security Number or Employee Identification Number

·       Department or School Name

·       Department or School Telephone Number

·       Account Number(s) and Transit Routing Number(s)

·       Voided Check(s)

Please note that you may elect up to two accounts for direct deposit. Once you’ve completed the form, you can fax the information to the Department of Finance, Metro Payroll Section, at (615) 880-2805. You can also mail, or hand deliver your form and voided check to the following address:

Please allow one-to-two pay periods for your direct deposit to be set up. Please note that direct deposit is handled directly by Metro Nashville General Government and not Metropolitan Nashville Public Schools.
